2010/2011 Athletic Physicals

The Flower Mound High School annual pre-participation athletic physicals will be held on
Saturday, June 5 in the FMHS gymnasium. LISD policy requires an annual UIL pre-participation
physical examination prior to participation in UIL athletics for all students entering grades 7-12.

You must bring a UIL pre-participation physical exam form with you to participate in this event.
The cost of each physical exam is $20.
